Extra Pages

If you need additional pages in your passport, simply bring it to the consular section during ACS business hours. A form DS-4085 must be filled out before we can add additional pages. Adding extra pages usually takes about one or two work-days and is done without any charge. All passports needing extra pages for children under the age of 16 are subject to the two-parent consent requirement. Parents who come in for extra pages must bring a signed notarized consent statement, or a duly completed and notarized Form DS-3053 from the other parent unable to be physically present before additional pages can be inserted into the minor's passport. In addition, the minor's original birth certificate and parents' identity documents are required.
