Lost and Stolen Passports

If you lose your passport or your passport is stolen while you are in Nepal, come to the Embassy during ACS working hours with the items requested below. Passports are almost never issued after hours. If an emergency requires you to obtain a passport on an emergency basis, contact the consular section at 400-7200 (after hours 400-7266).

To be issued a new passport after your passport is lost or stolen, you will need to fill out Form DS-11. Don't sign it until requested to do so by a consular officer. You should bring evidence of your citizenship, such as a photocopy of a prior passport, and evidence of your identity, such as a driver's license. If you cannot provide these documents, a consular officer will interview you to try to determine whether or not you are eligible for a passport. You may also wish to have a friend or relative in the United States contact Overseas Citizen Services (202/647-5225) at the State Department with your name and birthdate. Overseas Citizen Services may be able to help verify prior passport records and transmit them to the consular section.

You will have to execute an affidavit (Form DS-64) regarding the circumstances of your lost or stolen passport, which you will sign in front of a consular officer. You may be asked to provide a police report as well. If you would prefer to get the police report before coming to the Embassy, you should go to the Hanuman Dhoka police station on Durbar Square, and ask to speak to the Interpol Section.
