INSTRUCTIONS FOR THE MEDICAL EXAMINATION
Applicants for U.S. immigrant visas and transportation letters are required to have medical examinations performed by one of the two Nepali doctors listed below who have been selected as Embassy panel physicians. You must present your passport or other document of identity, two recently taken passport size pictures, and this instruction sheet to the examining physician in order to enable him to comply with the required procedures.

Effective February 20, 2008, the fee for the physical examination and tests for visa applicants 15 years of age and older is US$55.00, payable in Nepali Rupees equivalent. The exam will include tests for syphilis, immune deficiency syndrome, chest x‑ray, bacteriological test of sputum (on a need basis), blood group test, and a physical examination. Applicants who are pregnant should advise the panel physician, as they are exempted from the chest x‑ray.
The fee for children under 15 shall not exceed US$40.00, payable in Nepali Rupees equivalent; this will include a physical examination, blood group test, and may include a Mantoux test and/or a chest x‑ray depending on individual circumstances.
Should the general examination reveal the need for additional tests, the applicant will be responsible for the extra charges.
A blood test for antibody to the Human Immunodeficiency Virus (HIV) is required as part of your medical examination if you are 15 years or older. HIV is the virus that is the cause of the Acquired Immuno Deficiency Syndrome (AIDS). The test is not to diagnose AIDS, but to detect antibodies to the virus. If the test is positive, it does not necessarily mean that you have AIDS or will get it. The results of the test will be provided to a consular officer.
Applicants who suffer or have suffered from a chronic illness or have had a serious operation in the past should bring medical reports with them and present them to the examining physician.
United States immigration law requires that immigrant visa applicants obtain certain vaccinations (listed below) prior to issuance of an immigrant visa. Panel physicians will verify that immigrant visa applicants have met those requirements.
MUMPS, MEASLES, RUBELLA, POLIO, TETANUS AND DIPHTHERIA TOXOIDS, PERTUSSIS, INFLUENZAE TYBE B (HIB) AND, HEPATITIS B, VARICELLA, PNEUMOCOCCAL, INFLUENZA, ROTAVIRUS, HEPATITIS A, MENINGOCOCCAL, HUMAN PAPILLOMAVIRUS (HPV)
AND ZOSTER, HEPATITIS B VACCINE (UPTO 18 YEARS OF AGE) INFLUENZA VACCINE (CHILDREN AGES 6-59 MONTHS. ACELLULAR PERTUSSIS VACCINE (10-64 YEARS)
Applicants should make their vaccination records, if any, available for the panel physician’s review at the time of the medical examination. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirements. Only a physician can determine which of the listed vaccinations are medically appropriate for you, given your age, medical history and current medical condition. Please note that vaccination fees are not included in the fees listed above. Applicants will be responsible for all fees associated with each vaccination to be administered.
