Replacing Lost or Stolen Visas

If your passport containing a valid U.S. visa is lost or stolen, you are required to report the loss or theft to the Embassy. To do this, please complete and download the attached report (PDF 61.6KB) and either email it to the Embassy at consktm@state.gov as an attachment, or bring it to the Embassy on the day of your scheduled appointment.
Please ensure that your passport is not lying in a drawer somewhere before you submit the report.
